  • A copayment is a flat fee that you pay for medical services, regardless of the actual amount charged by your doctor or provider. This generally applies to doctors visits and prescriptions.

  • Coinsurance is the percentage of the medical or dental charge that you pay AFTER the deductible has been met.

  • A deductible is the amount you pay toward the medical and dental expenses each plan year before the plan begins paying benefits.

  • The Out-of-pocket-max is the maximum amount you will pay in coinsurance during the calendar year.

  • Changes to your benefits can only be done during Open Enrollment, unless you have a change in your status. If you have a change in your status you have up to 30 days to notify your employer to make changes. If you fail to notify within 30 days, you will have to wait until Open Enrollment.

  • A change in your status is when any of following life events affects you: marriage or divorce, birth or adoption, death of a spouse or child, a change in your work status, a change in your spouse’s work status that results in new benefits, change in your child’s eligibility for benefits, receiving Qualified Medical Child Support Order (QMCSCO). If you are not sure about a life event, contact your HR or Benefit Solutions team for support.

  • Your benefits will end on the last day of the month following the date of your termination/resignation.

  • COBRA is a Federal law that allows workers and dependents who lose their medical, dental, or vision coverage to continue any of these coverages for a special length of time by electing and paying for continuation of benefits completely out of pocket.
